Montgomery County Probate Court makes changes to Guardianship reporting requirements

The Montgomery County Probate Court will no longer send Guardian Report forms and Guardian Plan forms automatically to guardians on an annual basis via U.S. Mail. The Court now requires the guardian to download these documents from its website, complete them, and file them with the court on the anniversary of their appointment. 


To access the documents, visit the Montgomery County website at www.mcohio.org, and then:

  • Select "Departments O-Z" from the "Departments" menu tab, followed by "Probate Court."  
  • Choose "Guardianship" from the blue menu at left.  
  • Scroll down to the "Guardianship Filing Requirements" subheading, and click on the link labeled "Guardian of the Person Reporting Packet."

Guardians cannot waive the Reporting and Planning requirements.  If a guardian has no access to the internet and cannot get to a library to get online, then it is up to the guardian to contact the Court at 937-496-7041 and request that the Court mail a copy of the Guardian Report and Guardian Plan forms.
