Find a Provider

Individuals seeking adult day services, employment services and transportation from private providers can secure information about those providers in a number of ways:

  • Provider Profiles:  MCBDDS has compiled a Provider Profiles document containing information about day service, employment and transportation providers serving Montgomery County.  Information contained in the document was submitted by each provider.

  • Provider Location Map:  MCBDDS has created an interactive Google map showing the locations of all providers serving Montgomery County.

  • Provider Search Tool:  The Ohio Department of Developmental Disabilities maintains an online Provider Search Tool that helps families find private sources of day services, transportation, homemaker personal care and housing services. Instructions on using the Provider Search Tool and viewing DODD Provider Compliance reviews can be found in the attached document, titled Process for Reviewing Provider Compliance Reports Online.

  • List of Questions:  The Montgomery County Board of Developmental Disabilities has compiled a List of Questions you may to consider asking Day Service Providers when interviewing.

The Montgomery County Board of Developmental Disabilities Services does not endorse or recommend specific providers.  All providers have met the minimum requirements established by the Ohio Department of Developmental Disabilities, and are contracted with the Montgomery County Board of Developmental Disabilities to provide services.

All provider information is gathered directly from individual provider agencies, and is updated as we receive information.

The Montgomery County Board of Developmental Disabilities Services recently launched a new Provider Evaluation Toolto help families compare the services and programs offered by private provider agencies.  This two-page tool, which is offered in spreadsheet form atwww.mcbdds.org/providerevaluationtool, was assembled with input from providers and lists agencies in alphabetical order.

This spreadsheet covers a wide variety of categories, including art, autism, career discovery, fitness, high behavioral needs, incontinence, job coaching, job exploration, job placement, meals (for a fee or included, full assistance or partial assistance), mental health services, nursing (consultation and on-site), p.m. shift programs, recreation, social, supported employment, enclave-based employment, technology training, transportation (standard and specialized), volunteerism, and wheelchair accessibility.
