Report a MUI
Call 937-296-1235 - Montgomery County Board of DDS operates an MUI intake reporting line for all MUIs. This line is a voice mail and is available 24 hours a day, 7 days/week.
Email MUI@mcbdds.org to submit all Incident Reports (IRs) in conjunction with a call placed to the MUI intake reporting line.
Reporting Procedures
Physical Abuse, Sexual Abuse, Emotional Abuse, Neglect, Exploitation, Misappropriation, Peer-to-Peer Act, Failure to Report, and Unexplained or Unanticipated Death.
- Immediately (no later than 4 hours after discovery of the incident or allegation), the reporting call must be made to the MUI intake reporting line.
- By 3:00 p.m. the next business day, a completed Incident Report must be submitted to the MUI mailbox listed above.
- When a provider has reason to believe a criminal act has occurred, the provider will immediately REPORT TO / NOTIFY the Law Enforcement Agency having jurisdiction where the incident occurred.
- Notify Montgomery County Children's Services (937-224-KIDS) for all allegations of abuse and neglect of individuals under the age of 21.
- ALL ABOVE to be completed BEFORE reporting the incident to MCBDDS.
All other MUIs must be reported to the MUI intake reporting line by 3:00 p.m. the next working day (Monday - Friday).
MUI Reviews and Analysis
Agency providers must review MUIs annually for trends and patterns by January 31 and send the analysis report to the county board by February 28.
